Why did the women's gymnastics vault event get a do-over at the 2000 Sydney Olympics?

Answer The vault was 2 inches too low.

It was discovered that the vault was set to the wrong height after several of the gymnasts struggled with their routines, many landing on their, um, backsides. After the vault was fixed, those gymnasts who'd already performed were given the chance to try again. Pretty sure somebody lost his or her job on that one.
